L’objectif de ce travail est de proposer une méthode d’imagerie fonctionnelle aortique accessible aux cliniciens. La méthode combine la tomodensitométrie (TDM) et la mécanique des fluides numériques (CFD: Computational Fluid Dynamic) dans un processus quasi-automatisé, qui modélise l’aorte avec un minimum d'intervention humaine. Cette méthode a permis d’étudier une population de 52 cas (22 aortes saines, 30 anévrismes abdominaux sous rénaux) pour une meilleure compréhension des paramètres morphologiques et hémodynamiques de l'aorte. Nous avons pu démontrer la faisabilité de notre méthode de modélisation par l’enchaînement de l’analyse morphologique (Osirix, Pixmeo, Switzerland) et fonctionnelle (XFlow, Next Limit Technologies, Madrid). La mise en relation de l’exploration morphologique de l’aorte (diamètres, volumes) avec les résultats CFD (vitesse, pression, contrainte de cisaillement par segments aortiques) a permis d’émettre des hypothèses sur le remodelage de la paroi artérielle, le déplacement des endoprothèses et le risque de rupture de l’AAA. Ce travail propose un nouveau type d’imagerie fonctionnelle qui complète le diagnostic basé uniquement sur l’analyse des diamètres aortiques. La modélisation CFD basée sur la méthode Lattice Boltzman présente de nombreux avantages (rapidité, gestion des géométries, maillages, interface) par rapport à la méthode de CFD traditionnelle qui résout les équations de Navier-Stokes. Ces avantages ont permis à un clinicien autonome de mener à terme cette expérience scientifique.
The goal of this project is to propose a functional aortic imaging method accessible to clinicians. This method combines Computed Tomography (CT) and Computational Fluid Dynamics (CFD) in a quasi- automated process, with minimal human intervention. This method was used to study a population of 52 cases (22 healthy aortas, 30 abdominal aortic aneurysm (AAA) for a better understanding of the morphological and hemodynamic parameters of the aorta. We demonstrated the feasibility of our modeling approach by linking the morphological analysis (Osirix, Pixmeo, Switzerland) and functional analysis (XFlow, Next Limit Technologies, Madrid). The relationship between morphological exploration of the aorta (diameters, volumes) and the results of CFD (velocity, pressure, wall shear stress of aortic segments) allowed speculating on the arterial wall remodeling and the risk of AAA rupture. This project proposes a new type of functional imaging which complete the diagnosis based on the analysis of aortic diameters. CFD modeling based on the Lattice Boltzmann method has many advantages (speed, geometries management, meshes, interface) compared to the traditional method of CFD solving the Naviers-Stokes equations. These advantages have allowed the clinician to independently complete this scientific experiment.
